Stormers centre Shaun Treeby has been cited for his dangerous tackle on Blues flyhalf Piers Francis at Newlands.
The incident took place in the 47th minute of the match, which the hosts won 30-22.
Francis was knocked unconscious as a result of the blow and is expected to miss the Blues' next outing against the Chiefs.
Treeby was only penalised for the tackle, with no card – yellow or red – being issued.
Upon further review of the match footage, the citing commissioner deemed the incident had met the red-card threshold for foul play.
The case is to be considered in the first instance by the Sanzaar foul play review committee at 11am (SA time) on Sunday.
