Hilton Lobberts will start at blindside flank for the Cheetahs against the Sunwolves in Bloemfontein on Friday.
Lobberts is drafted into the starting XV in place of the injured Boom Prinsloo, who tore his hamstring to the bone ruling him out for the rest of the season.
The other two changes are in the front row, where Charles Marais replaces Ox Nche at loosehead prop with Maks van Dyk starting at tighthead ahead of Johan Coetzee.
In the only positional switch, midfielders Francois Venter and Nico Lee trade places with the captain moving to inside centre and Lee to outside centre.
Elsewhere, loose forward Oupa Mohoje is welcomed back via the bench, having returned from injury for the Free State Cheetahs in the Currie Cup last weekend.
